To finalize your application/intake for Looking & Feeling FAB, please adhere to the following steps:
Click the link below to access the application form.
After clicking the link, enter your full name and email address, then click Continue.
You will receive an email containing a password. If you do not see it, please check your spam folder.
Copy the password and click on “Open Form.”
Enter the password and click on “Open Form.”
Complete the form to the best of your knowledge, then sign the form and click “Submit Signature.”
Following that, click “Submit Form.” You will be prompted to confirm your submission; click “Submit” again.
Your application has now been submitted! You can expect a phone call from a Looking & Feeling FAB representative within 4 to 7 days.
Additionally, please download and print the Medical Release form for your medical care team to sign. Once completed, email the signed form to Holly@lookfeelfab.org.
